The following questions are designed to address and probe your thoughts on the personal and professional ethics of IVF based on your readings and research. It is not intended to be a debate about abortion in terms of pro-life or pro-choice; however, from a medical healthcare perspective, such thoughts may enter into this discussion.   1. Medically when does life begin? What does “initiating life” mean to you in a medical sense?
  2. With all the talk about the ever-increasing availability of in vitro fertilization techniques, one issue is almost never mentioned! What do healthcare professionals do with all those extra embryos? Please discuss
  3. If the couple divorces, who owns the embryos? Is there any child support obligations down the road?
  4. How do you feel about embryo freezing? How long should they be kept? And what happens to the ownership of the embryos if both parents have died?

Life and Death issues and Basketball Ethics

You are at a family care conference for one of your ailing patients to discuss end of life decisions for Billy Bob. What’s left of your patient’s family is present including ex-wife Angeline and all three of their adult children. For the sake of these scenarios Billy Bob is unable to be present for or participate in this family care conference.

Case 1
In some patients, pain becomes unbearable despite the aggressive deployment of standard medication. Sedation is the use of medication to induce sleep, as a temporary measure to relieve severe physical distress in trauma, burn, post-surgical, and intensive care patients. Sedation at the end of life is used to relieve intolerable pain and other symptoms when death appears imminent and when all other means of relieving the patient’s suffering have failed. The goal of sedation in such cases is keeping the patient comfortable. Unconsciousness is an unintentional but expected side effect of sedation. It’s not the goal to render someone unconscious but due to the sedation methods used it is often a side effect.

The Doctor turns to you and asks this question:
Do you keep a hospice patient overly sedated in order to allow the patient to breathe but in doing so the patient would be unable to eat? Sedation means that death would hasten but the patient would be comfortable albeit not be able to talk to family in their final days/hours. What are the ethical issues here and what would you advocate for?

Case 2
Same family care conference as above with a different scenario. Billy Bob is in full code status and is 98 years old and death will come in a matter of weeks for Billy Bob. The Doctor is deferring decisions to the family members rather than advocating either way for Billy Bob. You are observing what is going on. What do you do here? What are the ethical issues and what would you say to advocate for Billy Bob or do keep quiet and follow the Doctor’s lead?
