Place your name on the IAP form you create on a Word document.
Complete the Individual Action Plan using the guidelines on page 263 of the text and create a form (Word document) similar to the IAP form on page 265. This assignment is for three goals – which you have not yet accomplished and were identified in your autobiography. At least one of the goals you will use for this assignment is a current life and/or career goal, and at least one other goal you will use in this assignment is an educational goal. The third goal is your choice from either category.

Part I: Part one of this assignment will consist of ONE goal from your autobiography. You will receive feedback that should be incorporated into Part II/III of this assignment.
Part II/III: Part two and three of this assignment will each contain ONE goal from your autobiography incorporating feedback from part I.
Each goal should have at least 5 activities that will help you reach that goal. Each activity must be measurable so that it can be determined if that activity was achieved.
Example of a non-measurable activity: Joan will improve writing skills in one year.
Example of a measurable activity: Joan will write a clear, cohesive paragraph consisting of at least 3 sentences, including compound and complex sentences that are clearly related, increasing her score on the writing rubric from 25/50 to 35/50.

Every activity listed should have at least one person or information resource (be specific) that you will need to complete each activity.
Then, of course, write the order of activity completion and the date you plan on completing each activity (Hint: the order of activity should correspond to the date). Place a check-mark next to each activity if you have already completed that activity with the date completed.
