I chose the senior with dementia case study. This case student presented a patient with variety of risk factors. Given his diagnosis he in unable to communicate for himself and can articulate his needs or health issues. Looking at what the interdisciplinary team with discuss, there is a variety of possible health factors such as poor nutrition, medication reaction, increased agitation (if placed in a nursing home) and an inability to articulate any changes in condition. This vulnerable adult is dependent on those caring for him to provide for his care and meet his needs without truly knowing what those needs are. The case study provides a good example of how an interdisciplinary team can help in this situation as they can try to meet all of his health needs as best as they can identify.
The community resource that I found would be the Area Agency on Aging of the Permian Basin. While the home health agency can provide many of the same services, I think that it would be important to get the clients wife involved with the agency as it would be a good resource for any current or future needs. It is an accessible resource as they have both a physical location and a phone number to call so that their would-be multiple ways to contact them for their services. Often their case management service are free so it would be affordable and they would be knowledgeable in paying for additional services and resources to do so. The referral source would meet the clientâ€
s needs so it would meet the availability requirement. I think that the client would find the referral acceptable. In my past experience I have heard good things about this resource so I know that they would be able to meet the clientâ€s needs as well.
