Choose a client that can be interviewed (client must be cognitively aware and willing to discuss how their medications are being used). Please do not jeopardize your relationship with family or friends with this assignment. It may be difficult for you and for the family member.
2. The client must be taking at least 4 medications.
3. Research the clientâ€
s medical condition(s) and the prescribed medications the client is taking. Form questions that are not intrusive to interview the client to collect comprehensive subjective & objective data.
4. Use various resources such as the Nurseâ€s Pocket Guide, Pharmacology textbook chapters or the Drug Guide to identify at least 2 actual or potential nursing diagnoses, plan goals, nursing interventions and evaluation of response.
5. Use resources to develop a patient education/teaching plan for this client.
