Identify each area of weakness from modules 3 – 7 (transfer the table below to your paper and list the weakness in the completed packets columns)
For each area of weakness, identify 2 actions that will prepare you for the CAT. Your actions should be SMART (specific, measurable, attainable, relevant, time-based)
For each action, include a timeline and expected calendar completion date. Implementation of the action plan must be completed within 30 days of assignment submission. (You will be implementing this action plan in N455B.)
What 2-3 barriers might prevent you from being successful as you implement and accomplish the action plan?
What 2-3 corresponding behavioral changes can you make to assure your successful implementation and completion of the action plan? Be specific and reflect the reality of your individual life style (what works for you).
