The paper is the culmination of the interview of the agency, the community engagement assessment, and recommendations. A scholarly paper is in APA format and includes appropriate citations. Each student is to submit a scholarly paper. Use the following outline for the paper:
I. The Agency/Population Served
· State the agency name, individual representative.
· What is the mission of the agency?
· Describe the population served and services offered.
· What are some the strengths of this agency?
· What are some of the needs?
· What did you learn from this interview about this population or service?
· How does this information influence your future practice in the field?
II. The Community Assessment
· Explain what you learned by applying the Community Engagement Assessment Tool.
· What are the strengths in the community for the population of the agency?
· What is not available for this population?
· How would this impact your practice in working with this population?
III. Proposal/Recommendation
· From your professional lens, what do you see as a needed intervention for this population in the community?
· Why is this intervention necessary for your work with this client?
· What should be the goals of the intervention?
· How would this intervention be employed?
· What resources are needed to employ this intervention?
· Explain how you would advocate for this intervention.
IV. Your Practice
· Discuss what you learned from this community assessment.
· What did you find most surprising?
· How did this experience influence your future practice?
Note: The paper must specifically address how your area of health care expertise integrates with the information covered in this class, addressing each of the areas of community needs discussed.
The PowerPoint
Develop a PowerPoint about the findings of the community engagement assessment. The PowerPoint should include an artistic component and a well-written and organized presentation.
The Guidelines for the PowerPoint Presentation:
· There should be approximately 10-12 slides.
· The slides should be concise and easily readable.
· Video usage is limited to 5 minutes.
· Creativity is strongly encouraged
Your final paper MUST address all of the above and your PowerPoint must be designed as if you were going to present it.
You MUST also look at the community assessment tool. Make sure that you address ALL or as many of the items that you can find without conducting an interview with the agency.
