When it comes to providing patients with resources who have nonacute cardiorespiratory issues our number one goal is to get them back to living independent lifestyles. To do this first we need to look at what support system they may have and evaluate their health literacy. As nurses we also know that “the cardiorespiratory patient will have a variety of needs after being discharged from an acute care facility. This includes cardiac rehabilitation, pulmonary rehabilitation, or just the need for different types of durable medical equipment†(Johnson, A., 2018). “Many of the resources that must be considered to help the patient transition back to self-care include durable medical equipment, medication, transportation, living conditions, cardiac rehab, pulmonary rehab, and return to employment issues†(Johnson, A., 2018).
