select a patient that meets the following guidelines: presents with at least 3 chronic illnesses, currently on a minimum of 5 medications.(The 3 chronic illnesses will be COPD, Type 2 Diabetes, coronary artery disease.) With this patient you will develop the following parts below:

Relevant social history
Allergies (List all allergies and reactions)
Medication List (include prescriptions and OTC) List the reason why the patient has been prescribed these medications

Relevant Laboratory Data
Include any appropriate therapeutic medication levels that you would order if taking care of this patient
II.Collaborative Problem List
List the patient centered priorities according to the two disciplines-list the other discipline
