A Community Health Assessment or Community Health Needs Assessment is a state, tribal, local, or territory health assessment that identifies the key health needs of that area through data collection and analysis. It provides organizations with an overall assessment about the health status, needs, and issues of the community (CDC, 2018).
Environmental factors in relation to one’s geographic location affect the health of the community. When examining the environmental factors, it is important to determine the geopolitical location. A geopolitical location incorporates boundaries, climate, transportation, animals, vegetation, structures, geographical features, infrastructures, and homes. Geopolitical boundaries may be man-made or human made. They may consist of mountains and rivers or of zip codes and jurisdictions (Grand Canyon University, 2018).
A Phenomenological place is a psychological place or a place of relation. It is not of geographical location. This phenomenological place is the center of beliefs such as spiritual, cultural, history, values, similarities, or education. These similarities create a coexistence of belonging here (Grand Canyon University, 2018).
The nursing assessment, using a holistic approach, should identify appropriate health issues, local or global. As a nurse, it is important that we are capturing all of these important background questions that can identify potential needs or warrant further investigation. When I have an intake assessment, I want a complete picture of this patient; Background, travel, living arrangements, education, food, substances, animals, housing, how often they see a physician, and any previous illnesses. This information will open the avenues to identifying risks or additional screenings or interventions that are needed.
