Consider the scenario below:
Workers need to enter a below-ground, concrete-lined vault to repair a leaking valve that controls the flow of wastewater from several buildings at your plant. The opening for the pit is outdoors and secured with a 36-inch diameter manhole cover. The pit is 12 feet deep, and the walls are 6 feet x 6 feet wide. The valve is at the bottom of the vault. You opened the cover, tested the atmosphere, and found oxygen levels to be at 16%. No toxic substances were detected. After ventilating the space for 15 minutes, the oxygen reading reached 18% and remained at that level.
Address the following items:
- Using the flow chart in Appendix A of 29 CFR 1910.146, determine the classification of the space. Explain how you reached the decision.
- Based on your determination, develop an OSHA-compliant standard operating procedure (SOP) for workers to follow when entering the space to repair the valve. Include the citation(s) from 29 CFR 1910.146 for each step of the procedure.
Your submission must be a minimum of two pages in length (double-spaced). Be sure to include a title page.
We look at the General Industry standards 1910.141 through 1910.184 in this lesson. Confined Space is one area of great importance. Be sure that you know the definition of what a confined space is, and then understand what is needed to cause a confined space to become a permit required confined space.
One of the major issues with confined spaces is when the space has not been properly identified, persons enter the space, are overcome by a poor atmosphere, and rescues are attempted without proper consideration of the situation. 3, 4, 5 and even more fatalities can occur, all gainful attempts to rescue those inside the confined space, but all suffering from the same issue that caused the first worker to go down! Unfortunately, the story goes on and on, when simple compliance with the provisions of the standard would have prevented any deaths.
Lockout-Tagout is another concept explored in this lesson. This standard came about because so many people would be servicing or maintaining equipment when for some reason, the equipment began to move, catching the worker and causing injury or death. There are very definite procedures to follow with this standard. Be sure that you understand them.
