A man contacted the Aging Resource Center and stated that he has no food in the home. He is a 74 year old frail elderly man who suffers from congestive heart failure, hypertension, diabetes, and high cholesterol. His heart problems and diabetes has left him severely visually impaired and he has expressed concerns he is unable to complete his personal care and homemaking duties. He is in need of transportation to attend monthly physician appointments. He is a hoarder (inside and outside of home) and because of the condition of his home he is at high risk of being reported to Adult Protective Services. He receives $898.00 a month from Social Security which is considered the poverty level, and he has shared that he no longer has any money in his checking and savings account. He has full benefits Medicaid and they take care of all the medications he is taking daily for his health problems. What are the appropriate referral services and resources you would provide for him. Why do you feel these referrals are appropriate?
