Principalism is a concept made up of four ethical principles that outline the basics of bioethics. These principles include respect for autonomy, nonmaleficence, beneficence, and justice. Respect for autonomy can be described as respect for the decisions made by autonomous persons. Nonmaleficience means doing no harm to others. Beneficence describes the principles of preventing harm, providing benefits, and doing what is in the best interest of others. Justice describes fair distribution of benefits, risks, and costs (Beauchamp and DeGrazia, 2004).

 

I would personally rank the four principles as follows:

  1. Nonmaleficence
  2. Beneficence
  3. Autonomy
  4. Justice

I think as a society, we should “first do no harm” and take care of each other as fellow human beings. Secondly, I would rank beneficence, as this principle gives us all an opportunity to do no harm plus to do good for others. Autonomy gives us the freedom to choose what we wish to do and what good we choose to participate in. Justice ensures all humans are fairly represented and that everyone is a recipient of the beneficence of others. I think the Christian narrative would rank the principles in the order of nonmaleficence, beneficence, justice, and autonomy last. To live as Christ lived would be to put the needs of others before our own.

 

According to our lecture, principalism in the United States, particularly regarding bioethics, has been critiqued for raising autonomy to the most important principle on the list (Lecture 3, 2015). I can see where this concern is coming from, as American culture tends to get selfish and people forget to balance their needs with the needs of others. I can think of a personal example from working in the hospital that was most disturbing to me. We had a 23 year old woman on our unit with chronic GI issues that was well known to the entire hospital for being a drug seeker who tried to manipulate providers and exercised control by refusing aspects of her care – gait belts, lab draws, vital signs, you name it. Her refusal of an initial colonoscopy led to multiple infections throughout her hospital stay. I was frustrated one night because she refused to let me change the dressing on her infected IJ line, and my sentiment echoed that of all the nurses and doctors at the hospital: “what is she doing here if she will not let us help her get better?” I spoke with my nurse manager about this, and while she acknowledged my concerns, we could not force her to do anything she refused – we could only document it. This patient later overdosed on heroin that her boyfriend brought to the hospital for her and remained in the ICU for months. So yes, according to the principle of autonomy, this patient had a right to make all the decisions that caused her condition to worsen. I think American culture could use a little modification on the importance of autonomy. Although this woman was competent, her drug addiction heavily influenced her mental capacity. I struggle with the fact that “we could only document her refusal,” when she really should have had access to mental health services and addiction counseling. The hospital system could have done better to prevent harm and offer her the services she needed.

This was a tough discuss questions for me. Hopefully I answered it right. 

 

Creation: “According to Christianity, the Christian God is the creator of everything that exists” (Grand Canyon University, 2015).

 

Fall: “Sometime after the creation, there occurred an event in human history in which this created order was broken” (Grand Canyon University, 2015).

 

Redemption: “The rest of the story in the Bible after Genesis 3 is a record of humanity’s continual struggle and corruption after the Fall, and God’s plan for its redemption” (Grand Canyon University, 2015).

 

Restoration: “The final chapter of this narrative is yet to fully be realized. While God has made available a way to salvation, ultimately the end goal is the restoration of all creation to a state of Shalom. The return of Jesus, the final judgment of all people, and the restoration of all creation will inaugurate final restoration” (Grand Canyon University, 2015).

 

Every human on this earth is made by God. God sacrificed His son for our sins. God has a plan as to when a person will be get sick and even die. I like to believe everyone here on earth has a purpose. I find myself questioning if God is so powerful why does he let people die at a young age? I must deeply believe He has a purpose when someone is taken from earth way too soon.

 

I also read when a human get ill it’s so “His purposes might be fulfilled” not exactly sure what this means. Or “God has also used sickness as a judgment against people who were in rebellion against Him” in hopes that individual will turn to Him for guidance 

 

To find comfort and hope in the light of sickness one must turn to God for guidance and healing hands.

As noted in the Learning Resources, systems theory provides a meaningful and beneficial means of examining challenges in health care organizations. To do this effectively, however, it is essential to assess all system components, as some may be relatively healthy while others are problematic.

 

For this Assignment, you apply systems theory to the examination of a problem in a department or a unit within a health care organization. (Note: You may use the same problem you identified for the Discussion as long as it meets the criteria for this assignment.)

 

To prepare:

 

  • Review the Meyer article, “Nursing Services Delivery Theory: An Open System Approach,” in this week’s Learning Resources. Focus especially on the information presented in Table 1 (p. 2831) and Figure 2 (p. 2833).
  • Reflect on your organization or one with which you are familiar. Within a particular department or unit in this organization, identify a problem the staff is encountering.
  • Using Table 1 in the Meyer article as a guide, analyze the department or unit, identifying inputs, throughput, output, cycles of events, and negative feedback. Consider whether the problem you have selected relates to input, throughput, output, cycles of events, and/or negative feedback.
  • Think about how you could address the problem: Consider what a desired outcome would be, then formulate related goals and objectives, and translate those goals into policies and procedures.
  • Research professional standards that are pertinent to your identified problem.
  • Reflect on the organization’s mission statement and values. In addition, consider how addressing this problem would uphold the mission and values, while improving the organizational culture and climate. (Depending on the organization you have selected, you may have explored these in the Week 1 Discussion.)
