At the beginning of my career as a nurse I started working as a licensed practical nurse LPN 2023

At the beginning of my career as a nurse, I started working as a licensed practical nurse (LPN) at a nursing home. I used to work at least 72 hours a week with the same residents. Initially, when I witnessed death at work, it was very difficult for me because I had become attached to residents emotionally. I used to interact with the residents almost every day of my life, and I treated them like family. Later, after I had experienced many deaths, I had learned to be more compassionate, able to accept and tolerate delays without becoming anxious or frustrated, and I made sure I was providing the best care of the residents during the last stages of their lives. I have decided to make sure they have the respect, compassion, and dignity that they deserve until their last breath. I make sure that they are comfortable, clean, and dry. The most important thing is that they are not suffering.  I have held their hands after my shift and stayed at the bedside for the patient and the family. I understand now that it is not about me, it is about them. According to Domrose (2011), “studies suggest that nurses go through a unique grieving process when patients die, and how they manage this process is important to their well-being.

 

To answer the question of if it gotten easier or harder for me to accept the fact of death, my answer is “easier.” It is easier because I can talk to my co-workers, friends, and the chaplain at the hospital where I work, and I have experienced many deaths in the years that I’ve worked as a float pool nurse, so it is not a complete oddity to see death. Also, the hospital where I work offers a committee for debriefings after a difficult death or a sentinel event. I’d like to add that float pool nurses in particular are often assigned patients who are dying.

Case Study: Healing and Autonomy

Mike and Joanne are the parents of James and Samuel, identical twins born eight years ago. James is currently suffering from acute glomerulonephritis, kidney failure. James was originally brought into the hospital for complications associated with a strep throat infection. The spread of the A streptococcus infection led to the subsequent kidney failure. James’ condition was acute enough to warrant immediate treatment. Usually cases of acute glomerulonephritis caused by strep infection tend to improve on their own, or with an antibiotic. However, James also had elevated blood pressure and enough fluid buildup that required temporary dialysis to relieve.

The attending physician suggested immediate dialysis. After some time of discussion with Joanne, Mike informs the physician that they are going to forego the dialysis and place their faith in God. Mike and Joanne had been moved by a sermon their pastor had given a week ago, and also had witnessed a close friend regain mobility when she was prayed over at a healing service after a serious stroke. They thought it more prudent to take James immediately to a faith healing service instead of putting James through multiple rounds of dialysis. Yet Mike and Joanne agreed to return to the hospital after the faith healing services later in the week, and in hopes that James would be healed by then.

Two days later the family returned, and was forced to place James on dialysis, as his condition had deteriorated. Mike felt perplexed and tormented by his decision to not treat James earlier. Had he not enough faith? Was God punishing him or James? To make matters worse, James kidneys had deteriorated such that his dialysis was now not a temporary matter, and was in need of a kidney transplant. Crushed and desperate, Mike and Joanne immediately offered to donate one of their own kidneys to James, but they were not compatible donors. Over the next few weeks, amidst daily rounds of dialysis, some of their close friends and church members also offered to donate a kidney to James. However, none of them were tissue matches.

James’ nephrologist called to schedule a private appointment with Mike and Joanne. James was stable, given the regular dialysis, but would require a kidney transplant within the year. Given the desperate situation, the nephrologist informed Mike and Joanne of a donor that was an ideal tissue match, but as of yet had not been considered—James’ brother Samuel.

Mike vacillates and struggles to decide whether he should have his other son Samuel lose a kidney, or perhaps wait for God to do a miracle this time around. Perhaps this is where the real testing of his faith will come in? “This time around, it is a matter of life and death, what could require greater faith than that?” Mike reasons.

Just as pain is a subjective feeling and we cannot attempt to define someone else’s pain, someone else’s suffering cannot be defined. “They too are autonomous, and, if autonomy is as important as the argument claims it is, then their autonomous requests for euthanasia should also be honored, even if they are not suffering greatly” (Meilaender, 2013, p. 63).  Therefore, I do not agree that suicide should be considered a sin.  If a patient decides that they do not want life- saving treatment, we would not consider that suicide, and why not?  Because we have the right to choose what is best for us based on our culture, beliefs, feelings, needs, and backgrounds.  Euthanasia can be considered similarly.  We make people comfortable at the end of life with prescribed medications, hence ultimately diminishing their respiratory drives leading to death, however; this is not considered euthanasia.  It is the patient’s choice to forgo suffering. Meilaender backhandedly writes, “Christians are, I suspect, more likely to be drawn to the argument that describes euthanasia as compassionate relief of suffering” (Meilaender, 2013, p. 65).  I do believe that no one can truly know the suffering of another person; the sufferer is not making his fate, but in fact living his fate.

As an oncology nurse, I see my patients suffer for months to years on end.  They suffer through chemotherapies, radiation, and surgeries to fight for their lives, or at the least prolonging them.  They have no intention of killing themselves willingly are unwillingly.  When there are no more options and death is inevitable, a comfortable ending should not be taken away from them.  It is their right they have earned.
