Questions: As an NP student, needs to determine the medications for recurrent H. Pylori infection.

Chief complaint: “ I have recurrent H. Pylori infection”.

HPI: M.C. a 46-year-old hispanic female presents to the GI clinic for complaint of recurrent H. Pylori infection. She was treated about 2 ½ months ago with H. Pylori triple therapy and failed treatment. She has pmhx of dyspepsia, GERD.

She also indicates that she has noticed that her symptoms of dyspepsia are worsening for past 2 months. She has associated her symptoms with nausea, upset stomach with all foods.

Denies associated symptoms of hematochezia, melena, hemoptysis, abdominal pain, fever, chills, pain or any other symptoms.

PMH:

H. Pylori infection gastritis

Diabetes Mellitus, type 2

Surgeries: None

Allergies: NKDA

Vaccination History:

She receives an annual flu shot. Last flu shot was this year

Social history:

High school graduate, married and no children. He frequently eats out in restaurants. He drinks one 4-ounce glass of red wine daily. He is a former smoker that stopped 3 years ago.

Family history:

Both parents are alive. Father has history of DM type 2, Tinea Pedis.

mother alive and has history of atopic dermatitis, tinea corporis and tinea pedis.

ROS:

Constitutional: Negative for fever. Negative for chills.

Respiratory: No Shortness of breath. No Orthopnea

Cardiovascular: No edema. No palpitations.

Gastrointestinal: No vomiting. +Dyspepsia. + Nausea. No constipation. No melena. No abdominal pain.

Skin: No lesions. No rash. No itching.

Psychiatric: No anxiety. No depression.

Physical examination:

Vital Signs

Height: 5 feet 5 inches Weight: 140 pounds BMI: 31 obesity, BP 110/70 T 98.0 po P 80 R 22, non-labored

HEENT: Normocephalic/Atraumatic, PERRL, EOMI; No teeth loss seen. Gums no redness.

NECK: Neck supple, no palpable masses, no lymphadenopathy, no thyroid enlargement.

LUNGS: Lungs clear bilaterally. Equal breath sounds. Symmetrical respiration. No respiratory distress.

HEART: Normal S1 with S2 during expiration. Pulses are 2+ in upper extremities. No edema.

ABDOMEN: No abdominal distention. Nontender. Bowel sounds + x 4 quadrants. No organomegaly. Normal contour; No palpable masses.

GENITOURINARY: No CVA tenderness bilaterally. GU exam deferred.

MUSCULOSKELETAL: Slow gait but steady. No Kyphosis.

SKIN: Dry. Intact.  

PSYCH: Normal affect. Cooperative.

Labs day of visit:: Hgb 15.2, Hct 40%, K+ 4.0, Na+137, Serum Creatinine normal 1.0, AST/ALT normal. TSH 3.7 normal, glucose 98 normal

A:

Primary Diagnosis: Recurrent H. Pylori infection gastritis

Secondary Diagnoses:

Dyspepsia

Differential Diagnosis:

Peptic Ulcer Disease

Previous medication plan: two months ago and failed.

  1. Clarithromycin 500 mg po BID for 2 weeks
  2. Omeprazole 40 mg po BID for 2 weeks and then po daily.
  3. Cipro 500 mg po BID for 2 weeks

Plan: Tests

Pt had EGD done 2 weeks ago that showed H. Pylori positive gastritis in biopsy results.

Urea breath test 8 weeks after treat with H. Pylori medications. Pt needs to stop PPI’s 2 weeks prior to Urea Breath test.

Labs: No new labs are needed.  

Referrals: may refer based on effect of medication therapy given for 2 weeks.

Follow up: return to office in 8 weeks to reevaluate her symptoms.

An important step in the EBP process is reviewing the current body of literature to better understand the subject or topic in which you are interested. By conducting a review of the literature, you are building foundational knowledge about the topic; later, you can use this background to build new insights. Developing a strong grasp of a topic can only be garnered by taking the time to thoroughly search for relevant information and resources.

For this Discussion, you will practice searching the literature to find evidence on a specific topic.

To prepare:

  • Choose a simple search term(s) relating to a topic of your PICOT question.
  • Review the information on the evidence hierarchy discussed in Chapter 2 of the course text, in the article, “Facilitating Access to Pre-Processed Research Evidence in Public Health,” and in the multimedia presentation “Hierarchy of Evidence Pyramid,” found in this week’s Learning Resources.
  • Review the information on the Walden Library’s website, “Levels of evidence.” Take a few minutes to explore the different types of databases available for each level of evidence and focus on the meaning of filtered and non-filtered resources.
  • Conduct a literature search in the Walden Library on your selected topic using the databases that you reviewed. Use at least one database for each of the three levels of filtered information and at least one unfiltered database. Record the number of hits that you find at each level of the hierarchy of evidence.
  • Select one article from the results at each level of the hierarchy. Compare the articles based on the quality and depth of information. What would be the value of each resource if you were determining an evidence-based practice?
By Day 3

Post a summary of your search. Describe what topic you selected, the search term(s) that you used, and the number of results found at each level of the hierarchy. Compare the types of information found in the articles from different levels and the value of the information from each level. Highlight a useful tip that you could share with your colleagues about conducting an effective literature search.

The symptoms of chronic constipation include: abdominal pain, Passing out hard stools and straining when passing stool. Apart from the home remedies towards chronic constipation like taking high fiber food and a lot of water, medication is paramount in the treatment of chronic constipation. One of the medications that can be used in the treatment of chronic constipation is linaclotide which should be taken at a dose of 4mg per day for two weeks (Ford et al, 2014).

Soap analysis of the case study

Subjective

The patient presented herself to the clinic complaining of abdominal bloating, chronic constipation and incomplete defecation.She also indicated that the symptoms have been worse for the past three months. In addition, the patient argued that even after taking Miralax medication one packet daily for 8 weeks the symptoms are still there and there is no change.

Objective

The weight of the patient is 140 pounds while the height is 5 feet and five inches. The patient has no edema and no teeth loss seen. Glucose level is normal with no abdominal distention. The BMI of the patient is 31 obesity with a Bp of 110/70 T 98.0 po P 80 R 22.Hgb of the patient is 15.2

Assessment

Primary diagnosis indicates chronical constipation

Secondary diagnosis indicates IBS-Constipation and Incomplete Defecation

Plan

Plan medication for the patient is 4 mg of linaclotide daily for two weeks and tone compliance test to evaluate if patient has incomplete defecation. Biofeedback bowel retraining program should follow if test is positive to correct causes of incomplete defecation. The patient’s next visit is after eight weeks for checkup (Ford et al, 2014).
