(1)Long-Term Care in the U.S.

 

Determine the average daily cost of nursing home care in your state. Discuss the funding options (public and private) that exist to pay for long-term care. What policy and financing recommendations would you make to ensure that long-term care is available for everyone who needs the services in the U.S.? Please justify your answer.

 

(2) Rationing of Healthcare Services

 

Healthcare services can be rationed by governmental rationing or by allowing those who can afford the services to purchase the services. If you were the chairperson of a medical ethics committee faced with the decision concerning which of the following patients would receive a heart transplant, what decision would you make and what justification would you give to the families of each of the patients for making that decision?

 

Patient 1: A seventy-six-year-old nonsmoker with good pulmonary and renal function is a good candidate for a heart transplant, and his life expectancy without the transplant is one month. The patient has the resources to pay cash for the transplant procedure and follow-up care.

Patient 2: A forty-six-year-old unemployed auto worker with a history of alcohol use and smoking suffered a heart attack and will die within one month if he does not receive a heart transplant. The patient does not have health insurance, nor does he have the resources to pay for the procedure.

Over the next few weeks, you will explore many of the critical components of the Patient Protection and Affordable Care Act, also known as the Affordable Care Act (ACA). Each week, a different provision will be introduced. Your assignments will include researching the provision and explaining the changes that will occur with the ACA.

 

In Week 4, you will explore changes to Medicare included in the Patient Protection and Affordable Care Act.

 

Medicare

 

The U.S. created Medicare, a publically funded healthcare program, to assist people sixty-five years of age and older in obtaining healthcare services.

(2)Identify some of the changes that have occurred in the Medicare program since its inception.

(3) Describe the demographics of the population covered by Medicare and identify the number of people in the U.S. covered by Medicare.

(5)Research the ages of individuals in your state by reviewing information available on the website www.census.gov. How do you think the changes in Medicare will affect the citizens and legal residents in your state?

Consider the following three case studies.

 

Case Study 2:

 

 A 17-year-old girl comes to your office with a complaint of abdominal pain and missed periods. She thinks she may be pregnant. She requests pregnancy testing and does not want you to tell her parents if she is pregnant.

 

 

 

To prepare:

 

•Review this week’s media presentation, as well as “Developmental Management of School-Age Children” and “Developmental Management of Adolescents” in the Burns et al. text and the Schapiro article in the Learning Resources.

 

•Think about confidentiality laws regarding providers, school-age children, adolescents, and their families.

 

•Select one of the three provided scenarios. Reflect on the provider’s role and responsibility regarding confidentiality between the patient and the patient’s family in the scenario.

 

•Consider the appropriate way for the provider to respond and facilitate the care of the patient in the scenario you selected. Think about interventions and strategies that the provider should use to address the issues presented.
