Two of the healthcare delivery models that incorporate an interdisciplinary care delivery team is the patient care medical home and the accountable care organization. The patient centered medical home brings together the preventative and primary services as the foundation to the high quality and efficient health care. This allows more services to be provided all within the same day and in the same area. They also allow the patient to ask questions and have time to get them answered and availability of labs online.
Accountable care organization is a group of physicians, hospitals, or other health care providers that come together voluntarily to give coordinated high quality care to Medicare patients they serve. This is to avoid unnecessary or duplicate services. This allows the patient to get better care and to save the Medicare program wasted funding on repeated testing.
