There are many levels to the nursing practice, roles, and knowledge. Specialty nursing practice and advanced practice nursing are practices that are similar but very different in certain aspects. Even though both practices involve direct patient care, their roles in healthcare differ. To begin with, specialty nursing involves expanding knowledge in a specialized area, such as trauma nursing (Hamric, Hanson, Tracy, & O’Grady, 2014). Nurses who specialize in a particular area have extensive knowledge and experience in their given area of practice. On the other hand, advanced practice nursing not only involves specialization, but it also entails educational advancement. Advanced practice nurses must receive formal graduate level education while specialty nurses receive on-the-job training or hospital-based training courses (Hamric, Hanson, Tracy, & O’Grady, 2014). Because advanced practice nurses receive formal education, their level of knowledge is greater than that of a specialty nurse because their education covers more information and includes that of various specialties.

            The scope of practice of an advanced practice nurse and a specialty nurse also differs greatly. While both practices provide care to patients, advanced practice nurses are able to provide a broadened scope of practice while specialty nurses are focus on a specific area. The scope of practice from advanced practice nurses is standardized and allow for practice to be utilized to its fullest potential regarding education, training, and certification (Scope of Practice, 2016). For example, an advanced practice nurse is licensed to treat patients with various conditions, such as heart failure or diabetes, while specialty nurses work in a specific area, such as trauma or wound care. The population focus of specialty nurses is narrow compared to that of advanced practice nurses; however, advanced practice nurses may also be specialized in a specific population such as family and individuals across the lifespan, adult-gerontology, pediatrics, neonatal, women’s health and gender related, and psychological and mental health (Hamric, Hanson, Tracy, & O’Grady, 2014).  Also, advanced practice nurses not only have more education, but they are able to diagnose, manage, and treat patients using evidence-based practices while providing leadership and ethical decision making characteristics as opposed to specialty nurses (Hamric, Hanson, Tracy, & O’Grady, 2014).

            Lastly, credentialing and regulation go hand-in-hand when providing licensure to advanced practice nurses. According to the framers of the Consensus Model for APRN regulation, licensing was based on level of role and population focus (Hamric, Hanson, Tracy, & O’Grady, 2014). The level of role includes certified nurse midwife, clinical nurse specialist, certified registered nurse anesthetists, and certified nurse practitioner, and the population focus includes family and individuals across the lifespan, adult-gerontology, pediatrics, neonatal, women’s health and gender related, and psychological and mental health (Hamric, Hanson, Tracy, & O’Grady, 2014). Even though advanced practice nurses receive formal graduate education, which must be from a program accredited by the American Board of Nursing Specialties or the National Commission for Certification Agencies, they must complete clinical hours, as well as successfully pass an assessment of professional knowledge in their population focus (National Certification Expectations for Entry-Level NPs, n.d.). On the other hand, specialty nurse receives their licensure by completing courses and passing exams focused on their specialty.

Specialist, Nurse Practitioner, and Certified Nurse Midwife. The U.S. have their own individual nurse practice acts that discuss regulations for APN’s to include education, certification/licensure, and scope of practice (Watson & Hillman, 2010). Even though APN’s scope of practice may be different in each state APN’s can  universally can diagnose where specialty nurses can still only rely on nursing diagnosis. I do believe APN roles will became more standardized as the need for APN’s increases. According to Lathrop and Hodnicki (2014) if the Conesus Model for APRN Regulation were to be used then standardization for all APRN licensure, accreditation, certification, and education would be done through the legislation.Nurse Practitioners currently receive their credentialing by five different bodies dependent on the area of interest. These are the American Academy of Nurse Practitioners Certification Program, American Nurses Credentialing Center, American Association of Critical-Care Nurses, and Orthopaedic Nurses Certification Board (Watson & Hillman, 2010).

 

According to Hamric, Hanson, Tracy, and O’Grady, (2014) specialty nursing practice is nursing focused on a specific field where as “advanced practice nursing reflects concentrated knowledge in a specialty that offers the opportunity for expanded  and autonomous practice based on broader practical and theoretical knowledge base” (p. 113). Specialty nursing practices obtain their training through on the job training and/or their health care institutions through certification versus advanced practice nursing (APN) obtain their training through a formal educational graduate level program (Hamric et al., 2014). Watson & Hillman (2010) reported the required education level of an APN was going to be doctorate by 2015. Here we are in 2017 and it is still that of a Masters level.

I  believe APN’s will be required to have a Doctorate level of education in the near future due to the evolving health care needs of our population as well as due the increase in legal liability. Since the passage of The Affordable Care Act the need for Advanced Practice Registered Nurse with a Doctor of Nursing Practice has increased due to preventive health focus (Lathrop & Hodnicki, 2014). As we are all aware APN’s have historically been focused on primary care which puts us as the leader in this shift in health care. I believe this because he start as registered nurses and provided that direct patient care only as nurses we follow the treatment plan developed by the primary care provider and now as APN’s we can develop the treatment plan.

    Scientism is based on truth and evidence and accepted universally but religion is beliefs based on cultural backgrounds of any individual. People can be Christian, Muslim, Hindus or atheistic. Any issue can be right for one religion but same issue might consider wrong for other religion. For example marrying within families (cousin) is not acceptable in Hindus society while Muslim society marrying with cousins is acceptable.  There is still more issues that science is not still able to answer. The dilemmas between science and religion in health care setting is costly as it is related to life and death matter. Both aspects goes side by side.

     I believe having religious belief feed patient with healing power along with scientifically approved treatment. Respecting and acknowledging patient’s culture /religion help in maintaining their dignity.

    However people have their own point of view toward science and religion. Religion and science are often seen as opposite poles in our understanding of the world. And yet while religion and science are in some sense polar opposites, there is also much in common between them. Both are carefully crafted systems of thought, concerned with establishing the real truth behind the world we see in experience. (Lehar, S.
