The text “Called to Care a Christian Worldview for Nursing”, discusses how, as nurses, we have arrived at the point we are at now in terms of modern philosophies. “The spirit of service and compassion that once motivated nurses has evolved into a professionalism that demands power, status and appropriate compensation” (Shelly & Miller, 2006, p. 16).  This topic is the most interesting to me due to the fact that over the six short years I have been a nurse I have seen the population of nursing change.  I have been involved in a peer interview group on my unit for the past five years and have recently moved into a management position wear I actually hire employees.  I remember in the beginning of my experiences of interviewing new nurses, most had a story behind the interview question, “Why did you become a nurse?” I remember one new graduate nurse explaining that her mother and grandmother were both nurses and she aspired to be like them, caring, respected, smart, and making a difference with their work.  Another nurse explained that her husband had died of alcoholism and she watched as his nurses judged him and did not treat him with respect.  It was her goal to do better as a nurse and provide her patients with respect and the care they deserved no matter what their background.  Most recently I have seen a change in the reasons why nurses are choosing the profession.  The last interviewee discussed how the hours were great; “3 12s” and it works out great for her family life.  I have noticed an increase in the theme of the convenience nursing allows for people and less of the compassion or calling. 

The analysis of disease and healing presents interesting insight into the role of healthcare providers and spirituality. “They stand beside us, but they have not voluntarily shared our fate” (Meilaender, 2013, p. 9). The text describes the contradictory relationship between God working through doctors to treat disease and treating the disease may enable a false hope of invulnerability, therefore creating the inability to comprehend suffering.  I understand the reading to mean that there must be a balance between God (Health) and healthcare (health).  I agree with this inference for the fact that I believe that one’s mind, or relationship with God, must be as strong as the belief in the doctors to provide relief.  If one’s mind is not in the right place, the therapies will not assist in healing. “Spirituality enables the direct experience of connection between the individual and God, and can exist with or without an intermediary such as a religious institution” (Grossman, 2013, p. 1150).

The ICU that I used to work in also had a sepsis screening tool. However, this tool was a computer based program. It was linked with each patient’s chart. The screening tool would fire based on heart rate, blood pressure, and lab values (in particular WBC and coagulation times). 

 

My primary role during my time on the unit was the rapid response nurse. After the sepsis screening tool fired a pager would go off with the patient’s information (Name was not included, just the FIN and MRN numbers along with location of the patient). 

 

I feel it was a GREAT tool. Especially when you would go to the unit and get ABG results with a pH of 6.9 which did happen a few times. By far the benefit outweighs the risks, which in this case most of the “risk” was financial. 

The system had a lot of false alerts. Most of the alerts were from the dialysis patients and the OB patients in active labor. But we still went and saw every patient. The biggest downfall for the sepsis screening was probably the financial burden. I’m sure the program was expensive. I know a lot of staff hours were spent for the rapid response nurses to go and see every patient. But I personally have seen it save lives!

 

From a floor nurse standpoint, I think most of the nurses liked it and I’m sure the families were relieved that the hospital had many checks and balances just in case the provider or nurse overlooked some vital information or lab values.

As mentioned in the class lecture, “scientism is the belief that the best or only way to have any knowledge of reality is by means of the sciences (Moreland and Craig, 2003, pp. 346-350).”  The view of scientism appears to reject knowledge of beliefs held without scientific knowledge or support.  This would seem to include beliefs like religion.  Similarly, postmodernism seems to do away with many beliefs and views of people who are religious.  In a postmodern world there would be no universal religious or ethical laws, as everything is shaped by the culture one exists within (BBC, 2009).  For a variety of reason, including scepticism and disillusionment with religion in general, some have a tendency to choose science or other beliefs over religion. As a nurse who does not subscribe to scientism or postmodernism, I understand the tension that exists within our society and Western culture, but I don’t experience much internal tension in the matter.  In science, I see the foundation of the nursing profession.  I depend on science to help in our understanding of the human body and our understanding of the world around us.  The world is a better place as a result of scientific knowledge and breakthroughs that have occurred over the course of human history and knowledge should be something we continue to seek to better care for our patients and each other.  That said, science cannot answer everything.  Science does not provide answers to morals, ethics, and values that individuals and societies must find answers to.  In this way, for many, including myself, religion also helps us to understand the world around us, just in a different way than what science can offer.

What does nursing philosophy have to do with your practice and how you practice as an advanced practice nurse?

 

Philosophy, or a type of mindset, as discussed in our book (Butts & Rich, 2015, pg 24.), is the underlying logic and ethical background that guides nursing practice. Nursing philosophy is composed from all the reasoning, rationale, and knowledge instilled in the nursing student so that it is carried forward to project the scientific basis and values molded by clinical experience. Nursing theories are used to help explain reasons for cause and effect of human behavior. Both philosophy and theory are important to nursing, especially advanced practice nursing because of their ever growing presence in the health care world. As nurse practitioners are becoming more necessary to be primary providers, both in rural and urban populations, and both primary care and specialty settings, the philosophy of caring for people needs to reign firm and true.

 

An important part of my own person nursing philosophy stems from being raised in a home with morals such as ‘doing unto others as you would have others do unto you’. I know I am not alone on difficult days where you have to dig deep and remind yourself of why you became a nurse in the first place. My own nursing philosophy has a lot of emotional support and stems from the ideas that this particular patient is a grandmother, a sister, a loved one of someone else and deserves great care. I also am a champion for education, and providing the tools to the patient in order to help better care for themselves when they leave our care. In the article by Hountras (2014), she discusses the importance in her own guided practice, and about educating new nurses and being a mentor as they develop their skills. I agree with this also and how much of a positive influence nurses can have on new nurses, students and patients. According to Butts & Rich (2015) providing care, educating and progressing the profession of nursing are all encompassed in a necessary societal duty we have within our discipline.

 

A final philosophy I implement into my nursing practice is advocating for patient self care. This is a branch of the education I already discussed, however this aspect of it goes further to empower the patient, providing resources and community support groups among any other necessary resources to set up the patient for optimal success. Dorethea Orem’s self care model is one I have always been found of because it does not place the provider drivers seat that control the patient’s health. In an article by Thrasher (2002), she discusses how Orem’s theory model promotes self care so that the person (if capable) should engage in learning about new health issues, be motivated to participate in future preventative scenarios and invest in reaching realistic goals along the way. This gives the patient more control over their health and treatment, allows them to understand they are part of the preventative primary health care, and that we have some role and determination in our own outcomes.
