I NEED A POSITIVE COMMENT BASED IN THIS ARGUMENT..BETWEEN 120-150 WORDS

What nursing interventions are appropriate for Mrs. J. at the time of her admission?

1.Oxygen – increase O2 circulation

2.Administer diazepam or any anxiolytic as ordered. This will enable the patient to calm down

3.Low stimuli environment – pt is anxious

4.TED hose and SCDs – reduce edema, increase circulation

5.Non-slip socks, bedside commode, fall precautions – diuretic will have her up often to use the restroom; pt is high fall risk

6.Incentive spirometer – remove fluid from lungs to prevent/reverse pneumonia

7. Chaplin – further calm anxious pt

Drug therapy is started for Mrs. J. to control her symptoms. What is the rationale for the administration of each of the following medications?

Medications:

IV furosemide (Lasix): It reduces extravascular volume by inducing diuresis hence decrease preload and the work of the heart in pumping blood hence prevent progression of heart failure. That is Lasix is a diuretic that releases excess fluid, thus reducing strain on the circulatory system (Howard, 2001).

Enalapril (Vasotec): This medication is an antihypertensive which causes vasodilation of peripheral vessels decreasing afterload resistance hence effective in heart failure. Its help to lower blood pressure, increase cardiac output and exercise tolerance without changing the heart rate (Gierrthmuehlen, 2016).

Metoprolol (Lopressor-it’s a beta blocker and usually used to treat high blood pressure, chest pain, and heart stress after a heart attack due to its antagonistic effect on of beta receptors on the heart. It slows the heart rate, allowing the left ventricle to fill more completely. The slowing of the heart (or the conduction through the AV nodes) helps to regulate A fib. A slower heart rate will assist the pt to feel less anxious.

IV morphine sulfate (Morphine) is a narcotic analgesic use to manage pain. It also increases O2 supply to the heart while decreasing the heart’s O2 demand.

Describe four cardiovascular conditions that may lead to heart failure

High blood pressure- Uncontrolled BP is a high-risk factor for developing CHF. When pressure is blood vessels is too high the heart has to pump harder than normal to keep up, and over time it causes the heart to get weaker.

Nursing interventions:

Ensure all home medication is on the MAR. Verify the medications with MD and adhere to BP protocol ordered by MD.

Education is the key: Educate pt to be compliance with all medications ordered by MD.

Smoking is a major risk factor for the development of coronary heart disease. CAD is a leading cause of heart failure and also the death of some part of the heart muscle.

Nursing interventions

Educating on smoking cessation and the risk factors associated with it will help prevent heart failure and lung cancer. Nicotine patches is essential for all smokers. It assists in smoking cessation.

Obesity-: This is another problem that increases heart failure. Overweight’s cause the heart to work harder than normal and cause sleep apnea.

Nursing interventions:

Exercise and proper diet help manage obesity. Educating on the long-term effects of obesity and some ways to help with weight loss.

Sleep apnea-  Pauses in breathing can contribute to fatigue and cause high blood pressure and heart failure, diabetes, stroke.

Nursing interventions

Encourage patients to use their prescribed CPAP.

Educate on the importance of CPAP and risks of noncompliance.

Cardiomyopathy: That is enlarged heart muscles (either due to increased workload over time, or genetic abnormalities), require more oxygen and energy to efficiently pump blood through the heart (Ottaviani, 2016). Enlarged walls of the heart chambers also decrease the size of the chamber, which reduces the amount of blood that can fill the heart at any given time.

What can be done in the form of medical/nursing interventions to prevent the development of heart failure in each condition?

1.Monitoring blood pressure-this will prevent heart from overworking

2.Blood sugar monitoring and advice on diet to decrease blood sugar

3.Cholestrol level monitoring-to reduce chance of getting coronary artery disease

Taking into consideration the fact that most mature adults take at least six prescription medications, discuss four nursing interventions that can help prevent problems caused by multiple drug interactions in older patients. Provide rationale for each of the interventions you recommend.

1. Help and educate the patient on keeping an accurate log and list of all medications and herbal medication that the patient is on. This will assist in drug interactions.

2. Educate the patient on the importance of choosing one primary care physician to reduce polypharmacy.

3. Help and educate the patient on ensuring proper dosage and frequency by using medication organizer if patient unable to read.

4. Educate the patient on possible interactions of herbal medication and the need to inform the physician of all herbal medication.

5. Ensure the patient is educated on all new medications, indications, education, potential side effects, and potential interactions.

2023 Nursing Hello, I Need help again with my Assigment.. Plz and thank you

DQ 1 This week we discuss the levels of care in the U S healthcare system There are three delivery mode 2023

DQ 1:

This week we discuss the levels of care in the U.S. healthcare system. There are three delivery mode options. The delivery mode you will be discussing depends on the first initial of your last name. See the table below.

Guided Responses: Those who are assigned to Delivery Mode 1 will respond to those who responded to Delivery Modes 2 and 3, Delivery Mode 2 will respond to those in Delivery Mode 1 and 3, and Delivery Mode 3 will respond to those in Delivery Modes 1 and 2. Review the six concerns listed in each of your classmates’ discussions. Select two concerns within each post and examine how stakeholders in their specific delivery mode might address two of them.

Students with last names starting with A–H are assigned to Delivery Mode 1—long-term care—throughout this discussion; submit rebuttals to each of the other modes.

Delivery Mode 1: Long-Term Care (last names A-H)
Examine this mode of delivery and address the following elements in your discussion:

  • Describe the type of care this option provides. Consider the following:
    • Who is the target client?
    • What professionals work within that mode?
    • What does this mode offer in terms of care?
    • What are the expected outcomes?
  • Discuss at least two specific issues or concerns that affect the delivery of care for each of the following categories listed below (a total of six concerns):
    • legal and regulatory
    • ethical
    • social

Guided Responses: Those who are assigned to Delivery Mode 1 will respond to those responding to Delivery Modes 2 and 3, Delivery Mode 2 will respond to those in Delivery Modes 1 and 3, and Delivery Mode 3 will respond to those in Delivery Modes 1 and 2. Review the six concerns listed in each of your classmates’ discussions. Select two concerns within each post and examine how stakeholders in their specific delivery mode might address two of them.

Final Presentation: Part I

Look ahead at the Final Presentation directions in Week Five. The assignment this week is Part I of that presentation. For this assignment, Part 1, you will provide an overview of the U.S. healthcare system. Follow the instructions below to complete this part of your Final Presentation:

Introduction: Include a title slide. Next, include an introductory slide where you will identify yourself and describe the nature of this presentation.
Part I (of the Final Presentation): The U.S. healthcare system. Define the U.S. healthcare system by addressing the following:

    • Describe the history of the U.S. health care system. Include at least three of the revolutionary factors (e.g., the teacher’s union in Dallas, Texas; The American Medical Association; Civil War, first Marine Hospital, etc.). Chapter two in our textbook discusses the evolution of our healthcare system and is a good resource for this part of the presentation.

    • Identify at least one major development from each of the following: financial, legal, ethical, regulatory, and social [e.g., consumer demand] that transformed the system into what it is today. You may want to revisit the healthcare timeline.

  • Differentiate the stakeholders and their roles (e.g., healthcare professionals, clients [patients], health insurance organizations, government, colleges, and health care agencies
    • Include the positive and negative contributions of how they affect the health care system).

Creating Part 1 of the Final Presentation for the Week 3 Assignment
Part 1 of the Final Presentation must:

    • Be 5 to 7 slides (excludes the title slide and reference slides) and be formatted according to APA style as outlined in the Ashford Writing Center.

  • Be presented using Microsoft PowerPoint slide presentation.  You have a choice of one of the following options for adding the narrative to your presentation. Option 1 is using voice or Option 2 is adding speaker notes to each slide.
      • Option 1: Recording your voice on each slide. You can view the tutorial using this link: adding and recording voice to each slide. You can also click on the following links to view the Accessibility Statement and Privacy Policy. If you choose this option, you must include your APA formatted citation in the speaker note section of the PPT as well.

        • Note: if you use the voice option, make sure your recordings are clear and concise. It is also important to know that adding voice narrations that are too lengthy and adding graphics may cause your PPT file to be very large (megabytes). You may have trouble loading it to Waypoint. Revisions may be necessary if this occurs.

        • Speaker notes are the typed notes that appear below the slide that complement the presentation slides. Whereas the slides will have short bulleted items, the speaker notes will be more detailed. They are essentially what the presenter would say during the presentation to explain each of the bulleted points on the slide. Therefore, it is important that the speaker notes are concise and detailed when explaining the bullet points.
            • It is recommended that PowerPoint Slides contain no more than five bullet points and should not contain more than 5 – 7 words each. Do not type paragraphs or long sentences on the slide. The information that explains each bullet point is conveyed via speaker notes or by recording your voice to each slide.

          • It is required that the Microsoft PowerPoint presentation include the use of a voice or speaker notes. If you choose to use the voice capabilities within PowerPoint, you must include your citations in the speaker notes for each slide.

        • Include a title slide with the following:
          • Title of the presentation
          • Student name
          • Course name and number
          • Instructor name
          • Date submitted

        • Address the sections in the order outlined above (Introduction, Part I, II, III, and IV).

        • Present the issues with critical thought.

        • Use at least two scholarly sources (none of which should be media commentary, at least one should be from the Ashford University Library).

        • Document all sources in APA format (including graphics, charts and pictures that may be used within the presentation). Wikimedia Commons is a recommended source for creative commons images (to view the privacy policy, click here).

        • Include a separate reference slide formatted according to APA style as outlined in the Ashford Writing Center.
