Consider all aspects of patient safety, both universal (i.e., hand-washing) and patient-specific (i.e., stemming from the disease process), and provide a rationale. • Q-2: Describe the nature of your communication with the patient as well as the inter-disciplinary team (IDT). Specifically, what therapeutic communication techniques did you utilize when interacting with Ms. Jacobson? What did you include in your SBAR report to the IDT members? • Q-3: Describe the focused assessment you completed (health history and physical exam) and provide a rationale for your decisions.
