Written Report Instructions:
Please read the Dupont Investigation Report and review the links found on the Written Report file on Canvas.
Your report is focusing on the 2010 event that resulted in the death of an employee exposed to Phosgene gas.
What I am asking for is a written report summarizing the events leading up to the death, a summary of the accident and a description of the OSHA requirements for each of the violations.
I then want you to conclude the report with written recommendations on how this accident could have been prevented.
This written report is taking the place of the class presentation.
