and Management

Think of the managers you have reported to thus far in your career. Now consider the people you have worked with or know of that you would consider leaders. Based on these experiences, what would you surmise about the responsibilities of managers and leaders and about the distinctions between these two categories in health care settings?

This week’s Learning Resources classify management and leadership (which are often confused in everyday discussion) and explain their significance for health care organizations. As you advance professionally, it is critical to understand the distinctions between management and leadership and how you can apply this knowledge for increasing effectiveness in your workplace.

To prepare:

Review the information in the Learning Resources.

Conduct additional research on your own and select at least two current, credible sources that contribute to your understanding of management and leadership.

Reflect on how the roles of management and leadership differ in supporting the organization to set and achieve goals.

Drawing upon specific examples from a current or previous practice setting, bring to mind someone who seemed to be a leader but not a manager and someone who seemed to be a manager but not a leader (generally speaking, or within a specific circumstance). Be prepared to support your assessment with specific behavioral descriptions found in the literature.

Social determinants of health are related to the environment in which people live, learn, work, and play that affects a multitude of health risks and outcomes.  According to the CDC (2014), these conditions are shaped by the amount of money, power, and resources that are present and determined by policy choices. When these conditions are out of balance, living a healthy lifestyle such as a well-balanced diet, getting physical exercise, obtaining vaccines, and seeking care when ill, are challenging.  This is where community nurses knowing their resources is crucial to help support their patients with deficits.  This unequal opportunity is influenced by poverty, lack of access to health care, inadequate education, shame, and racism (CDC, 2014). Social determinants play a role in the risk of illness and a wide range of health issues including diabetes, heart disease, and obesity.  From a global standpoint, there is a vast difference in developing countries who are suffering from poverty.  “Poorer countries have higher rates of death, disease, and disability” (Maurer & Smith, 2013).  The children endure malnutrition and premature death.  Illness not seen in places like the United States are widespread in the poor nations which effects life expectancy. 

The communicable disease chain refers to infectious diseases that develop from the interaction of agent, host, and environment.  To be precise, “transmission occurs when the agent leaves its reservoir or host through a portal of exit, is conveyed by some mode of transmission, and enters through an appropriate portal of entry to infect a susceptible host” (CDC, 2012). There are many opportunities for nurses to help stop the spread of disease and provide the proper education to patients.  The first step nurses can take to stop spreading is by interrupting this chain by properly washing their hands which are a fundamental principle of infection control.  This should be performed in all required situations such as before any aseptic procedure, before and after handling any patient, touching soiled items, after using the restroom, and before and after handling food.  Other examples include assessing your patient daily for the need to continue use of a Foley catheter which is a source of infection and staying up-to-date on your vaccines.  Also, remaining at home when sick and covering a cough to avoid spreading.

Discussion: Health Care Policy

In this week’s media presentation, Dr. Trautman, Dr. Wakefield, and Ms. Coyle discuss how public policies initiated at the national or state level ultimately influence what occurs at the institutional and local level of nursing practice and health care delivery. Health care policy usually is developed to address health care cost, quality, or access, or a combination of the three. Due to the nature of their interaction with patients, nurses are well situated to be effective, knowledgeable advocates for their patients.

In this week’s Discussion, you will examine how current policies impact how you as a nurse provide health care. You will then select an issue of interest and determine how you could advocate for policy in that area.

To prepare:

  • Select one public policy that currently is impacting you and your practice. Consider the following:
    • What health care driver was the policy designed to address: cost, quality, access, or a combination?
    • Does the policy appear to be achieving its intended results? On what data are you basing your assumption?
    • What have been the effects (adverse or positive) of this policy on health care cost, quality, and access?
    • How is this policy affecting your nursing practice?
  • Next, select a health care issue—something you see or experience on a daily basis—about which you would like to influence a practice change through the policy process.
  • Review the Learning Resources, focusing on Kingdon’s Model. How would you utilize this model to guide your policy development?

Post a brief description of a public policy that is having an impact on your practice; summarize your analysis of the policy using the bulleted list above as a guide. Then, summarize the issue you would like to influence though a change in public policy, and outline how you would utilize Kingdon’s Model to guide your policy development.
