Assignment: Evidence-Based Project, Part 3: Advanced Levels of Clinical Inquiry and Systematic Reviews

Your quest to purchase a new car begins with an identification of the factors important to you. As you conduct a search of cars that rate high on those factors, you collect evidence and try to understand the extent of that evidence. A report that suggests a certain make and model of automobile has high mileage is encouraging. But who produced that report? How valid is it? How was the data collected, and what was the sample size?

In this Assignment, you will delve deeper into clinical inquiry by closely examining your PICO(T) question. You also begin to analyze the evidence you have collected.

To Prepare:

· Review the Resources and identify a clinical issue of interest that can form the basis of a clinical inquiry.

· Develop a PICO(T) question to address the clinical issue of interest you identified in Module 2 for the Assignment. This PICOT question will remain the same for the entire course.

· Use the key words from the PICO(T) question you developed and search at least four different databases in the Walden Library. Identify at least four relevant systematic reviews or other filtered high-level evidence, which includes meta-analyses, critically-appraised topics (evidence syntheses), critically-appraised individual articles (article synopses). The evidence will not necessarily address all the elements of your PICO(T) question, so select the most important concepts to search and find the best evidence available.

· Reflect on the process of creating a PICO(T) question and searching for peer-reviewed research.

The Assignment (Evidence-Based Project)

Part 3: Advanced Levels of Clinical Inquiry and Systematic Reviews

Create a 6- to 7-slide PowerPoint presentation in which you do the following:

· Identify and briefly describe your chosen clinical issue of interest.

· Describe how you developed a PICO(T) question focused on your chosen clinical issue of interest.

· Identify the four research databases that you used to conduct your search for the peer-reviewed articles you selected.

· Provide APA citations of the four relevant peer-reviewed articles at the systematic-reviews level related to your research question. If there are no systematic review level articles or meta-analysis on your topic, then use the highest level of evidence peer reviewed article.

· Describe the levels of evidence in each of the four peer-reviewed articles you selected, including an explanation of the strengths of using systematic reviews for clinical research. Be specific and provide examples.

By Day 7 of Week 5

Submit Part 3 of your Evidence-Based Project.

Assignment Practice Scenario

Conduct nursing research studies involving clinical practice problems

Your Chief Nurse Executive (CNE) recognizes you are pursuing your BSN and has asked you to be a member of the nursing evidence-based practice (NEBP) committee. She has advised the members that the purpose is to conduct nursing research studies involving clinical practice problems. Increasing nurses’ awareness of quality and safety concerns will help develop an attitude of inquiry and asking questions (Sherwood & Barnsteiner, 2017).

Baccalaureate nurses “collaborate in problem identification, participate in the process of search, retrieval, and documentation of evidence, and integrate evidence using credible databases and internet resources” (AACN, 2008, pp. 16-17).

The CNE has advised all members of the following areas of quality and safety clinical practice that each committee member needs to consider and choose one as a priority area for conducting a research study. Sources of research problems come from nursing clinical practice (Houser, 2018):

QualitySafetyPatient satisfaction
Patient educationFallsNon-pharmacological pain managementErrors (any). Examples include:

  • Medication
  • Patient identification
  • Communication

Readmissions
Length of stay
CostsHealthcare Associated Infections (HAIs). (Any type)
Examples include:

  • Central Line Associated Blood Stream Infections (CLABSI)
  • Catheter Associated Urinary Tract Infections (CAUTI)
  • Ventilator Associated Pneumonia (VAP)
  • Surgical Site Infections (SSI)

SepsisDischarge Teaching/Coaching
Transitions of Care/Level of CarePressure Ulcers
Skin Integrity

Your CNE has requested that you are to select one area and complete the PPL Worksheet using that topic. The committee will collaborate and decide as a team which nursing clinical problem area will be the priority focus of their next research study. Thank you for your valuable contributions to this important NEBP committee.

Discussion Question 2: Decision Making Decision making is a skill and a competency nurse leaders must use in every act, word, and deed. A decision involves making a choice between alternatives, and it really is an act of choosing the best plan of action given the information at hand at the time the decision is made. Nurse leaders must be decisive as a decision leads to a definitive plan of action. Some decisions must be taken immediately and others may allow a bit more time. An important concept to remember with respect to decision making is that not making a decision or taking any action is also a decision. Considering this, nurse leaders must be attentive to every detail. For example, if there is no decision to honor the organizational policy regarding unacceptable behavior or action of an employee, the unacceptable behavior or action is then condoned or perceived as acceptable and may occur again. Nurse leaders’ decisions drive actions! Using the readings for the week, the South University Online Library, and the Internet, respond to the following: Describe the importance of making data-driven decisions. Explain a decision your current or former nurse leader made and how this decision impacted the healthcare organization. As a nurse leader, you have received information about an executive administrative decision from the board of trustees that you believe is likely to be unpopular. Formulate a plan to share this decision with the nursing staff. Comment on the postings of at least two peers. Week 9 – Assignment 1 Discussion Discussion Question 1: Patient Safety High-performing teams in healthcare organizations are the key to success in creating cultures of quality and safety. A high-performing team that works collaboratively enhances communication and patient safety. Healthcare teams that are not high performing and do not work well together are actually a liability in patient safety. For example, it is apparent to patients and their families when healthcare teams are not working collaboratively and communicating with each other. Kaiser Permanente has developed a framework of communication that healthcare staff can use to communicate about their patients, called the situation-background-assessment-recommendation (SBAR) technique (Institute for Healthcare Improvement, n.d.). This technique can be used to foster a culture of teamwork and patient safety. Using the readings for the week, the South University Online Library, and the Internet, respond to the following: Discuss the role of the nurse leader in fostering and promoting initiatives related to patient safety. Examine the relationship between patient safety and high-performing interprofessional healthcare teams. Discuss how the use of communication strategies like SBAR improves the high-performing interprofessional healthcare teams. Comment on the postings of at least two peers. Evaluation Criteria: Discussed the role of the nurse leader in fostering and promoting initiatives related to patient safety. Examined the relationship between patient safety and high-performing interprofessional healthcare teams. Discussed how the use of communication strategies like SBAR improves the high-performing interprofessional healthcare teams. Justified your answers with appropriate research and reasoning. Commented on the postings of at least two peers.
