Week 1 Discussion

Nursing revolves around the patient, the nurse, the patient’s health, and the environment. Therefore, these four concepts are known as the metaparadigms of nursing. There are many additional concepts that come to mind when I think of nursing. As a registered nurse, I feel as if collaboration and advocacy are two additional concepts that are relevant to my personal practice.

Collaboration is defined as “working with others in a way that promotes/encourages each person’s contributions toward achieving optimal/realistic patient/family goals” (Butts & Rich, 2018). In my opinion, working in the healthcare field truly requires amazing teamwork. A patient cannot heal purely by the medications the nurse gives them every morning. They must walk with physical therapy to get stronger, they must get their breathing treatments from the respiratory therapist, their labs have to be drawn by the phlebotomist, the radiologists must read their x-rays, and the physician must speak to/evaluate the patient each day to ensure that his/her goals are being met. Thankfully, I work in an environment where collaboration is welcomed. As the nurse, physicians often ask my opinion, as I have a closer relationship to the patient. Collaboration is most successful when there is active communication regarding patient care, and the correct resources are used to optimize patient outcomes (Butts & Rich, 2018).

Advocacy is the second concept that I feel is relevant to my personal practice. According to Butts and Rich (2018), advocacy is defined as “working on another’s behalf and representing the concerns of the patient/family and nursing staff.” On a daily basis, I find myself advocating for my patients. I do this by keeping the entire healthcare team informed, providing assistance or getting them in touch with the correct people to get them assistance, and providing professional nursing care. Working on an orthopedic unit, I most often find myself advocating for my patient regarding their pain. As nurses, we are taught that “pain is what the patient says it is.” If I feel as if my patient does not have adequate pain control, I am quick to call the physician. By advocating for my patients, I want to ensure that they get the proper care to optimize their health and outcomes.

4. Discuss aspects of the population health concern not being addressed despite the efforts of the partners involved. Based on interviews, what are the system level gaps? Why is it still a problem?

B. Outcomes Identification
1. Explain the desired outcome(s) or goal(s) for improvement related to the health concern. Overall goal to improve health concern. Make sure it is in line with the national standard.

Note: This outcome or goal should be in alignment with Healthy People 2020 objective.

C. Planning
1. Create a nursing action plan to address the identified population health concern.
a. Recommend two population-focused specific objectives. (mini goals)

Note: The recommended objectives should be measurable ones.

b. Recommend two population-focused specific nursing interventions to improve the health concern. What is needed to reach each objective? Change within the community.

Note: Use the attached �Minnesota Intervention Wheel� as an aid in selecting the broad areas for nursing action. Select primary and secondary prevention activities only. For example, explain how you and other nurses might work with the community and your chosen target population of interest to improve the health concern. (File Attached)

2. Discuss potential public and private partnerships from your field experience that could be formed to implement your recommendations from parts C1a and C1b. Various organizations and key leaders and what they bring to the table

3. Create a specific timeline for your population-focused nursing interventions.

D. Evaluation
1. Explain how you would evaluate the effectiveness of the nursing action plan created in part C
a. Identify the measurable tools necessary to perform the evaluation. Must be measurable. (ie. Survey, BMI, etc.)

F. Conclusion
1. Reflect on how your perspective of the community�s health and the national, state, and local efforts toward a healthier population has changed as a result of your fieldwork.

Patient education goes hand-in-hand with improved patient outcomes and quality of life. Educating the patient is an integral part of the nurse’s role in patient care. In order to ensure the success of the patient teaching plan, the processes involved in developing a nursing patient teaching plan should be centered around the patient and inclusive of the family.

Patient Assessment

This step requires the analysis and organization of information about the patient. Determine the patient’s level of understanding about her disease, injury or condition.

Use the information gathered about the patient to judge how well she will be able to understand and apply what is taught to her.

Be sure to factor in cultural considerations that are specific to that patient such as ethnicity and religion. It will be necessary to factor in any information you have about the patient’s level of development as well. It may be necessary for the nurse to motivate the patient in order for her to learn. Ability to learn is affected if the patient is too tired, in pain, upset or distracted.

Planning

Formulate patient goals. Take the patient and the family’s point of view into consideration, as their input will have an effect on their outcomes. Patient outcomes and motivation are directly associated with the amount of input they have in determining their goals. The goals must be realistic and able to be achieved by the patient.

Identify how the teaching plan will be carried out. Implement the plan according the patient’s learning style (audio, visual, or kinetic) as opposed to the nurse’s teaching style.

Develop nursing goals based on desired health outcomes. They should be patient-centered and based on health outcomes for the patient’s health status and quality of life.

Determine Method of Implementation

The method of implementation of the nursing patient teaching plan should be specific to the patient’s learning style and desired outcomes that were identified.

Prepare to document patient responses and level of understanding as the nurse perceives them while the teaching plan is implemented.

Prepare to document any other relevant information that will assist with the evaluation of learning.

Formulate Evaluation Process

An evaluation process will need to be formed in order to assess patient outcomes. The following specific elements of patient learning will need to be evaluated: patient knowledge, behavior, attitude, and skills. The evaluation should include a review of the documentation that details patient responses and level of understanding that were recorded during the implementation steps.

The evaluation plan should include observations of the family’s understanding of what will be required of them for the patient to achieve desired outcomes.

The last key element of the evaluation plan is the nurse’s recommendation for follow-up procedures to be used in the event that the patient’s learning was not sufficient to produce desired outcomes.
